ThorWorks fined for mixer accident

PERKINS TWP. — The Occupational Safety and Health Administration (OSHA) fined a Perkins Township manufacturer for an accident last year that left a man injured. An OSHA inspection at ThorWorks on Campbell Street found three violations, which resulted in a fine of $16,878, according to OSHA records. The fines come after a July 15, 2019, incident where a male employee fell into an industrial mixer containing tar and became entangled. After about two hours, Perkins firefighters freed the man and a medical helicopter took him to a Toledo hospital for treatment. ThorWorks reported the incident to OSHA, who did an inspection and found three safety standards not followed, according to OSHA records: • General requirements: "Access and egress. The employer must provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces" • "Duty to have fall protection and falling object protection" • "Training and communication" relating to "the control of hazardous energy" The fine was paid off on Nov. 12, 2019, OSHA records state.
