The role of primary care

As people get older, it can be easy to avoid thinking too much about health problems. Some people follow the idea that "ignorance is bliss." While this may feel easier in the moment, failing to take meaningful interest in your health can lead to poorer health and less independence later in life. Primary care is changing. Today it is more of a partnership between patients and health care providers. It is no longer just, "I'm the doctor, do as the doctor orders." Additionally, your primary care provider may not be a doctor. The primary care provider may be a nurse practitioner, a doctor of nursing practice or a physician assistant. No matter who provides your care, understanding their role can have a major effect on your future health and quality of life. Your health and independence are not only your provider's responsibility. Patients also have an important role. Both the primary care provider and the patient must work together, especially if the goal is to continue living independently and avoid assisted living or nursing home care, as one continues to grow older. If older adults truly want to stay healthy and remain independent as they age, one of the first steps is understanding what good primary care looks like. In the article, "What Quantifies Good Primary Care in the United States? A Review of Algorithms and Metrics Using Real-World Data," the authors explain that good primary care should do four main things: • Be easy for patients to access • Provide consistent care over time • Meet many different health needs • Help coordinate care between different doctors and services The real value of primary care happens during the appointment itself. Older patients who know how to come to a primary care provider appointment "prepared" often receive better care and have better health outcomes. But what does it mean to come to a primary care provider appointment prepared? From 1:30-3 p.m. on June 11, Serving Our Seniors will host a forum at the Erie County Senior Center, 620 E. Water St., Sandusky, called The Role of Primary Health Care and How It Works. Speakers are Dr. Mitchell Rice, of Firelands Health, and Lisa Miller, of the Erie County Health Department's primary care center. The discussion will cover: • The modern role of primary care and family medicine • How older adults can prepare for appointments • What patients can do to help their doctor, nurse practitioner or physician assistant provide better care unique to you • Ways older adults can improve their chances of aging well and staying independent A minimum of 30 RSVPs is needed to host the forum.
